Qualtrics Survey Getting Blocked by GC Admin #1260

Description

The Qualtrics survey getting blocked in GCNotify admin. We could report it via Google Analytics or mixpanel (or others). It wouldn't be difficult to add JS for the missing loaded component.

Acceptance Criteria

[ ] Qualtrics survey should load in GC Admin

QA Steps

[ ] Tested in staging

Check with Jeana and if they're still getting responses, than this isn't really a problem. Maybe in the future we use a survey button that links to Qualtrics or another form for feedback

Qualtrics library probably gets loaded externally so we could try and include their JS library as part of our packages instead of loading it externally

We should timebox effort on this to 1 day to explore if JS would fix for GC Admin, if not, Icebox
