Rebrand #98

This PR replaces all occurrences of Pulse in the repo with new names based on the discussed naming.

Note @szinck1 The environment variable names will have to change in the deployment. PULSE_ENV -> TRACKER_ENV PULSE_MONGO_URI -> TRACKER_MONGO_URI

This PR addresses most of https://github.com/cds-snc/pulse/issues/42
