Integrate opencds-rest-service into ice #35

create top level project inside ice/opencds-rest-service with copy of opencds-rest-service
add to opencds-parent to point to the opencds-rest-service.
get the latest changes to EvaluateResource and friends from rckms-dss (fixing MAJOR Invocation.Builder bug, thanks Blake).

@darylbc take a look when you get a minute - I've ported the opencds-rest-service to live inside ice, and I also ported Blake's latest improvements. I didn't merge directly into fb-expose-version, as it's kinda bigger.
