add documentation for JSON policy sets #121

Description of changes:

We decided to release the new policy set JSON format as part of 3.3.0, so this PR pulls back the documentation for this feature that was added to the staging-4.0 branch. It also reorders the "document history" and "language history" sections as discussed offline.
