Unable to create form #1

Looking forward to using this, but I'm getting a generic error of "An error occurred while trying to save the form." when trying to create a form. I'm using Revo 2.1.1-pl. It appears to be related to the form name, but I've tried "Contact Form", "ContactForm", "test" and even blank. None worked. Any ideas?

Same issue here. Found out it wasn't creating the database table. That might be what's happening here.
