Port of @ceramicnetwork/streamid to provide StreamID and CommitID

Why a port? Couldn't we just use the @ceramicnetwork/streamid package directly?

If we want this SDK to work independently of js-ceramic, I don't think it should have any dependency on the @ceramicnetwork packages, I thought that was kind of the point of having a new SDK?

It's also more convenient to have a dedicated package here to add necessary functionalities, such as the codecs.

I could add @ukstv but I think I've seen the limit to 1 reviewer before as well, maybe because it's a private repo still? I think we're more limited than public ones.
