v5 upgrade #71

Most of the work here is related to updating the test fixtures and test code, and a lot of time spent on bugs obfuscated in part by the necessary "manual" changes to the test fixtures (aka is the bug because the test fixture was updated incorrectly or because the code is wrong?).
