CDC: Add data citation information #161

This might not be profile issue, depends if the citations will be about the record in CDC or referencing to the original (meta)data source.

If profile issue, then either: codeBook/docDscr/citation/biblCit (if referencing metadata) or codeBook/stdyDscr/citation/biblCit (if referencing study)

It is planned that CDC would have citation export feature without the need for citation metadata from SPs.
