was forked and reused elsewhere?
was used to build something new?
received contributions in the form of comments, issues, or pull requests?

https://github.com/CapitalJS/capitaljs/pull/11 CapitalJS merged PR with a new formula from a Code for Philly meetup attendee who I introduced to the node/JS to get them up and running with this project.

eRegulations has a handful of public contributions and was forked by 18F to the eregs org.

Our Jekyll theme DOCter is used by several government agencies and others to host documentation/information, including 18F, the City of Austin, Georgetown University, Department of the Interior, and the City of Detroit Health Department.

Not exactly code re-use, but in March 2016 a professor at Oklahoma State assigned his class of 200 Agriculture Economics students to write a class paper on planning for their retirement, and required them to use /retirement/before-you-claim/ to do so. From his feedback:
