Feedback modules #481

We're using feedback across an increasing number of pages, and there are a lot of small variations, plus some bigger ones. Here are the examples I found:

Ask CFPB feedback

In addition, the Ask CFPB feedback module, but no others, records the yes/no response without needing the user to click the submit button:

The second and third examples are, I believe, the current feedback module as built in wagtail, and the OaH child page is a variation on that. This seems like a good time to document our standard for feedback modules, and discuss whether we want to make any changes to the wagtail module.

Other feedback examples from Paying for College. Two different tools with different entry page language, that lead to (I believe) one common form page whose title changes based on entry page. Neither of them are in Wagtail.

One issue we know already know of is that the note telling people not to include sensitive information needs to be included whenever we have a free response comment box. There also should be a plan to scrub PII from the feedback if users end up including it.
