Update "Affiliate Entity Information" to "Immediate Parent Entity" & "Top-holding Parent Entity"

Per RIG guidance, we should change the "Affiliate Entity Information" to "Immediate Parent Entity" & "Top-holding Parent Entity".

"109(b) uses parent or top-holding parent entity. I think we need to use those terms for the sections. Affiliate is too broad and has other meanings, so it isn't quite accurate."

This needs to be updated on three pages:

• "View your Financial Institution Profile"
• "Update your Financial Institution Profile"
• "Sign and Submit"
