Simplify Create App dialogues #68

The current Create App dialogue lists available templates and after the user selects one a new small dialogue prompts the user for an app name. This dialogue often appears in a random location.

A better workflow would be

  1. Include the app name input text box on the initial template list dialogue
  2. When the user selects a template populate the app name text box with a default name. This would be especially convenient in a teaching environment
