Grading may produce a duplicate face in rare cases #10

A duplicate face was generated by the grading algorithm as documented under https://sourceforge.net/p/mesh2hrtf/wiki/Basic_HRTF_NumCalc_Simulation/#msbe-issue and https://sourceforge.net/p/mesh2hrtf/tickets/36/

@timonpalm - do you currently have time to look into this?
