Remove debug statement in index_fasta #114

Removes debuggin print statement from IndexFasta. See issue #113

Also tweaks the help statement to remove a incorrectly escaped format string.

Erm.... Not really sure what is happening with the tests here.

The strange errors seem to go away with openssl version 1.

Now there are errors with the tests for bam2bam. Not sure how to solve them, sorry.
