Default page at mythweb//episode/ #21

If you navigate to mythweb/episode/ you get an error. I have set a default to 
"modules/episode/show.php"

I have also tidied up the set-up/install instructions to set menu link to 
"episode" rather than "episode/show".

Patch attached.
