make the TVWish links configurable

At the moment, I am not using TVWish.  I would like to have the option to not 
display the TVWish buttons.  This patch will allow the user to make this 
configurable.  The patch has it defaulted to display the TVWish buttons.  To 
hide TVWish, change tvwishHide = true in handler.php.

Chris this patch is fine with the exception of one thing.  Alec had added a 
patch a couple of days ago that adds tvwish to the mythweb welcome menu.  This 
will also need to be hidden when tvwish is hidden.

Thank you Chad.  I didn't think about the welcome page.

I'm having some issues disabling the tvwish link in the welcome menu.  I will 
think about this a little more and then submit the patch at a later time.
