I think customer has either old version of the extension or cache problem.
Since quite some time FQ products are not allowed to be added from product list.
Changing price option recalculates all displayed prices.
Our extension does nothing to catalog/product/list.phtml file to cause any
fatal errors

I have checked this locally and it works as I say.
This has been checked on our test servers as well.
I cannot say what version they use because the account given has no access to
magento connect manager.
Also this site is kind of messed up - categories links don't work. Can't really
replicate the issue.

I think the customer posted this problem has a mix of cache issues plus he
needs to save FQ once more.
Cache is a problem always when upgrading an extension. Should always be
refreshed.
FQ are set to add a required option flag to product when they are saved. This
is what prevents adding product from category page to cart directly. But for
this to work, the product has to be saved at least once after FQ extension is
updated.

I did a fresh installation of Magento 1.5.0.1 + fixed quantities 0.3.5b (beta).
Same problems occur (excluding the fatal error).

To prevent products being added to cart directly from category page, they
should be saved after installing 0.3.5 or above. I have added a check in save
process to confirm if there are FQ used for the product and if yes, then
product is marked as having required options. If product has required options
it cannot be added to cart from categories page.
I would recommend either resave all products that have FQ or make an export of
the FQ then remove them from the products and import them back.
