Form Creation Error #6

What steps will reproduce the problem?
1. Install Grails 2.2.1, create a new app "demo"
2. Install form builder (followed the instructions in the getting started page)
3. resolve plugin dependencies
4. Run grails app 

What is the expected output? What do you see instead?
The form designer UI not showing up. Please see the attached document.

What version of the product are you using? On what operating system?
Windows 7, Grails 2.2.1

Thanks for your interest to the plugin and sorry for late response. I no longer 
actively support this plugin and busy with my new job recently.
