Separate Service Fabric from chaostoolkit-azure extension #50

Separate the Service Fabric functionality. The Service Fabric functionality is moved to the chaostoolkit-service-fabric extension.

The Servcie Fabric capability has moved to its own package https://pypi.org/project/chaostoolkit-service-fabric/
