Stop using _options.colors #50

Changes:

do not pass colors as a separate option to pods, extend series with color field instead
update @chartwerk/line-chart and @chartwerk/bar-chart to 0.2.0
update dist

Other changes:

merge methods extending series into extendGrafanaSeriesWithChartwerkOptions
rename colors import to grafanaColorPalette
add some TODOs
