Clarify documentation on measurement reports #32

The API call for /records/{RECORD_ID}/reports/minimal/measurements/{LAB_CODE}/ (documented here) does not make it clear what can be used for LAB_CODE.

We mention this data model in the "Processed Medical Reports" section of our docs, but not in the "Core Data Models" one. With the update to 0.5, we need to revisit our core models and decide which to deprecate, and better document the others.
