Same problem, for me wifi is unable to reconnect after deep sleep mainly if
there was previous connection to different wifi, e.g., I connect to wifi at
home then I go to office, connect to wifi, put phone to deep sleep, wake up
from deep sleep, I have to reboot phone to get connected again.
I noticed there could be some problem with settings - I disable wifi in
settings, I quit settings, run settings again and wifi is back enabled, it does
not preserve disable state.
I have R13.
I got the following when I tried to reinsert bcmdhd module:

Still having this problem in R15.
Trying to reflash R15 and hoping the problem is solved

Same behaviour at least from R8 - wifi seems to be on, trying to get list shows
me empty "list" of wireless APs... And even trying to disable Wifi does not
disable it. Workaround is to reboot phone, then you are able to use WIFI...
As far as I remember, in R5 this problem did not occur as I used it longer time
without wifi problems. Full wipe done everytime.

the issue is the same without internet commander app, the phone has wifi after
some time of sleep(random) the phone shows no wifi , the wifi is ON, try tu
put it OFF it wont go to off, after some time *5 seconds maybe it goes ON
again. r19

I have the same issue even in R30 :( After deep sleep WiFi is not working, I
can't disable/enable it. It doesn't find any access point. In System
settings/Wi-Fi I see "Searching for Wi-Fi networks...". Only one solution -
reboot.

Also get the same issue in R30 with full wipe - also get the same issue with
R30 and using kangbang kernal, and also after changing radios a couple of
times. Definitely seems to be if wi-fi is connected and the phone goes into
deep sleep the wi-fi state gets stuck and only a reboot fixes it...
