merge w/ koop-heroku-edn #1

@chelm @jgravois what do you think about merging this repo w/ https://github.com/jgravois/koop-heroku-edn and moving it to http://github.com/koopjs? Would be good to have a canonical heroku quickstart. Extreme bonus points if it's able to be deployed with a "Deploy to Heroku" button.
