Cannot uninstall BoxOffice #24

What steps will reproduce the problem?
1. I downloaded BoxOffice from my iPhone on 7/27/2008
2. The app continually crashed
3. After numerous sync's of my iPhone, BoxOffice still does not appear in my 
iTunes App section 
so I can remove it upon sync

What is the expected output? What do you see instead?
BoxOffice never appears as being installed on my iPhone, but it is!

What version of the product are you using? On what operating system?
BoxOffice 1.0
Mac OS 10.5.4
iTunes 7.7
iPhone 3G 2.0

Please provide any additional information below.

You can either:
a) install 1.2 (you should see an update for it in the appstore)
b) press down on the application icon for 5 seconds.   the icon will start 
wiggling.  click the 'x' on the icon to 
uninstall the app from teh phone.
