Addresses (does not close) #310, just this comment, which is about a) lazy-loading above the fold, and b) lack of image dimensions on some images

I'm sorry I haven't had a chance to make much progress on this. FYI I'm strongly considering significantly refactoring things and building upon the excellent work of https://www.npmjs.com/package/@hyas/images -- it does many of the same things we're trying to do in a more robust way.
