Changes / fixes

Addresses item 4 in #361:
- Adds GitHub issue forms for bug reports, new features, and documentation changes. The forms also prepopulate the labels and add prefixes to the titles.
- Adds a GitHub issue config file for the template chooser.
- Updates the PR issue template (unfortunately forms can't be used for PR templates unless you have a paid repo).

n/a. Unfortunately there's no real way that I know of to preview issue templates without merging the PR.
