Azure IoT Hub Support #111

The following PR adds Azure IoT Hub as a backend for the bridge. The Loraserver component that subscribes/publishes to the Hub will utilize Azure Event Hubs.

There is an extra bit about the documentation for the use of Azure IOT Hub (https://www.loraserver.io/lora-gateway-bridge/integrate/) , but maybe @brocaar can provide better guidance here.
