sub districts #185

I have some problems with this form pattern from https://wordpress.org/plugins/states-cities-and-places-for-woocommerce/.I can clear with states and places but sub districts. Could you suggest me the methods to add sub districts to the my website. Is anything wrong with my states-cities-and-places-for-woocommerce.php file?
