Looks like caption and subfigure (which loads caption unless given the argument caption=false) are incompatible with revtex4-1 but your use of subfigure and \ContinuedFloat) require these. I messed around with this for a few minutes, but it's not clear there's a simple fix, so I'm going to punt on this.
