Update documentation to reflect pypi upload and API #7

The cohortfinder readme should have the following updates:

  1. Installation can now be performed via pip install cohortfinder
  2. Provide information for using the CohortFinder API. See https://github.com/choosehappy/HistoQC/blob/6b2eeb23899d7c3ea403a4fad4dd1fd889a7afaa/histoqc/ui/HQC_html.py#L56 for reference
