Creating new version for implementation of death primary key

I think the omop,v5 model and version problems are straight from the v2.2 files and don't actually cause problems, but I think you should fix them while you're making this new version anyway...

There's just the one file with the wrong version, still.

Any hints on which file you are referring to, I checked the files on my end and only v2.3 is represented.
