Updating for OMOP v5.1 #165

... and also including a fix to allow the DDL for pedsnet 2.5.0 to be generated by the data-models-sqlalchemy service.

Aside: CircleCI is building when a PR is made, but the results are not injected into the conversation.

The build failed, but from looking at the ddl output under the build artifacts at circleci, I can see that the only errors are associated with omop 5.0.0. These are most likely pre-existing and of great vintage.

I'll create an issue about this but merge the PR.
