added service worker, updated manifest, more icons #34

Updated the manifest with new icons, and features to make this a PWA. Made site available offline (should add an ajax script to tell user they're offline)

@saxobroko - just curious; why were some of the existing files deleted?

to maintain consistency in the manifest, it also keeps the code clean. i renamed them to shorter names. For example, favicon-16x16.png is just 16x16.png
