Date of birth selection in submerchant form #291

DESCRIPTION: When selecting the month or year the date gets deselected. Move the submerchant form to a separate view so that all the relative code is in one place and there is only one template. HOW TO REPRODUCE:

  1. Try registering availability for a new user.
  2. Try filling the form and pay attention to the date of birth input.

Was not able to reproduce, and found separated sub-merchantregistration view in the project. I believe that this issue is solved.
