Process payment #41

At the earliest opportunity we should be able to process payments. At v0.1 that is likely to be for donations or 'pay-what-you-want-or-zero' type offerings.

So for MVP0.1, instead of a 'donate' button, we will put a link to this questionnaire: https://docs.google.com/forms/d/1VrUfHZ3oVFByCeZf19CNl_OY0po9gTuMlKIT20rKY48/viewform The link should go on the last couple of lessons, and could also appear everywhere once 50% of the lessons have been visited.
