Additional fixes needed for roundcube v1.3 and Calendar Server 9.2 #100

Implemented additional fixes to make the carddav plugin work nicely again with roundcube v1.3 and Apple's Calendar Server 9.2. This is implementation has been tested on Debian Buster and used version of Roundcube and Calendar Server are part of the distribution.
