Settings screen blank #1

After creating a form for use with this gravity-forms-zendesk addon I tested it and nothing happened on my Zendesk. I noticedI had two feeds for some reason. As I deleted the first one the zendesk settings screen is showing up as blank. It just has the title "Gravity Forms Zendesk Add-On" and nothing else.

Is the addon supposed to work and how do I reset it now so I can get back the settings screen?

I'm on Gravity Forms 1.9.13.

Ok, I found the settings screen by going through the settings link for this plugin on the WP pugins page.

But no ticket appears in Zendesk. Looking at the code it does not look like this is implemented yet?
