[x] polish the PDF a bit (layout, graphics... make it shiny)
[x] align the fields on the bogen with fields available in the software; change in either of them; keep the ORDER as well
[x] finally add most recent PDF to application

looks good so far. keep it simple (need to print it at home).

think about adding fields from old myshiatsu app.
