Update design for how sort function handles multiple data sources

Sort should work for all cards across multiple sources.

i.e., If you sort conditions by name for OHSU and VA it should show like:

• Cardiovascular disease - OHSU
• Cardiovascular disease - VA
• Diabetes - OHSU
• Diabetes - VA

Not: OHSU CVD Diabetes

From Michelle: When I tested the sorting feature with a secondary data source (in this case the VA sandbox), it returned all of the OHSU entries in alphabetical order followed by all the the VA entries in alphabetical order. My understanding was that the purpose of these features was to integrate the data together.
