Expand the list of lab results in MyCarePlanner #435

Currently, there are only 9 lab result types includes in the Tests tab, based on 9 value sets from the original NIH CKD TEP. There are now many more value sets in the MCC FHIR IG. Add additional value sets and results to display in the app by updating CQL logic.

All value sets from MCC eCare Plan IG are now included, except 2 that are value sets for panels, not individual tests.

82 different labs are now available to be reported in Tests tab of MyCarePlanner.
