Allow creation of new CBO user login for CBO Referral App

As a new CBO user, I want to create an account that allows me to access the CBO Referral App so I can manage referrals.

Acceptance criteria:

• "Create an account" screen and flow.
• New user is approved by admin.
• New user is added to Azure Active Directory.
• New user can use their new login credentials to log into CBO Referral App.
