If multiple explosions hit a human form Lycan, and bring his hp down to 0, on the same frame, then the transformation can be bypassed. Shrapnel Shot will occasionally do this, due to the fact it fires six separate projectiles. But it also works with Bertha projectiles, as displayed here.
I am not sure about other hits that aren't explosions, as I have not managed to get multiple of those on the same frame.
