Fix configure bug, version number handling changes

Ensure we don't check for FOM ever if using --disable-app
Move all version number info from acvp_lcl.h to acvp.h
Change how version strings are defined so we only need to update version number #defines in one place (Still also needs to be changed in configure.ac for now, may fix it so it all references configure.ac down the road).
