(@adam3smith, I generally never edit _journals.tab for the major publishers to introduce corrections, since such changes are easily lost when we get a new version of this file delivered to us; typically better to do this in one of the accessory files)

(P.S. Revisiting this PR as I'm tracking down and deleting some unused branches)

I agree in general (hence the use of extra and skip for the actual corrections here), but I think this is an exception: elsevier-vancouver is simply the correct vancouver style to use in the Elsevier list and any updated list should use that across the board rather than just for the styles that existed in the previous list.
