Add "none" option for 'and' attribute #403

Tasks:

  1. [X] moved two separate duplicate "and" patterns into a separate one
  2. [X] add "none" as option
  3. [X] run formatting
  4. [ ] test

OK, change made, but still needs testing and review.

The biggest consequence of the first change I made is just that the doc examples are specific to one context (names), but not the other (labels).

I think that's a small price to pay for enhanced maintenance.

Am also a little unsure of the "and" pattern name; might not be clear enough?
