Update the "Add Inventory Item" Form for New Technician Workflow #243

This PR updates the "Add Inventory Item" form to use updated fields and submit to a new scene and view through the Knack API. It also removes "Condition" from the "Add Inventory Item" form and adds "Source" and "Comment".
