Create a HTML page that offer the user and entry point for the functionality of Adding a new Driver to the system

@Ricciareactor - Describe below the look and feel or attach a wireframe to this ticket:

Eg.

Look:

Name input field
Surname input field
By default the Save button will be grayed out.

Functionality:

By default the Save button will be disabled.
When the user filled all the mandatory fields, the Save button is enabled
