Add github issue templates #8

I thought it might be a good idea to use Issues Forms:

It is also easier on the issue requester because it can prompt them for the necessary information.

@NickLarsenNZ I think this is a great idea! Is there a way to preview what it looks like in the UI?

Hi @ckampfe, I don't think there's a way to preview before they're added unfortunately.

I have made similar ones here: https://github.com/nerdswords/yet-another-cloudwatch-exporter/issues/new/choose
