Lots of blood and sweat needed for this one as the extension was in a bit of a state. Tried to improve things as I went along but had to draw the line somewhere. Some improvements:

Saner routes (eg no / in page parameter)
Consolidated assets in a single place
Fix form styling for Bootstrap 3
