Race and Ethnicity detail in sdtc:raceCode vs just raceCode #115

See the C-CDA_R2-1_CCD.xml file's values for tags raceCode and sdtc:raceCode. The first has a value for just "white" 2106-3, and the second has "Hawaiian or Other Pacific Islander" 2076-8".

I believe our code just checks the first, but it should be verified
We should pick up the second because it seems more detailed, definitely something CLAD and AoU would be interested in.
